Request to View, Update, or Erase your Personal Data

In response to the new General Data Protection Regulation (GDPR) laws put into effect on May 25, 2018, we offer you a simple way to request to view, update, or delete any of your personal data you have provided to us. If you wish to request any of these actions, please fill out the form below. Know that you will not be able to see this information immediately when you hit the Send Request button, but an email will be sent to the email address entered on this form with a response about what was requested.
